As you fill out the section below, please keep in mind that SML implements a 2 hr minimum on every job. Your hourly rate should be based on that minimum.
After hitting submit, your application will be sent to our vendor team who will review the details you provided. If your application is approved, SML will send you a blank copy of a W9 and a Preferred Payment Method Form. Both must be
filled out in order to move forward with enrollment.
As soon as our team receives your completed W9 and payment form, we will send you a Welcome Letter. This letter willconfirm that you have met SML’s requirements and have been added to our vendor database.
